The abnormal enlargement of the thyroid gland is called GoitreGoitre causes in normal-functioning (euthyroidism), or under-active (hypothyroidism) or over-active thyroid gland (hyperthyroidism). 

Here are the following that how goitre causes
 
-> Iodine deficiency – The important function of the thyroid gland is to concentrate iodine from the blood to make thyroid hormone. If there is no sufficient iodine, the gland cannot make enough thyroid hormoneConsequently, the pituitary gland in the brain senses the low thyroid hormone level and sends a signal (TSH) to the thyroid. This hormone stimulates the thyroid to produce thyroid hormone and to grow in size. 
 
-> Hashimoto’s thyroiditis – This is an autoimmune disease, where the immune system destroys its own thyroid gland. As the gland becomes more damaged, it is less able to secrete thyroid hormone. The pituitary gland senses a low thyroid hormone level and secretes more TSH to stimulate the thyroid. This hormone stimulates the thyroid to produce thyroid hormone and to grow in size. 
 
-> Grave’s disease – In this disease, the immune system produces a protein called thyroid stimulating immunoglobulin (TSI). TSI stimulates the thyroid gland to enlarge and to secrete more thyroid hormones.  gland to enlarge and to secrete more thyroid hormones. 
 
-> Multi nodular disease – In this case a single large nodule or multiple smaller nodules are found within the thyroid gland, which cause thyroid enlargement. 
 
-> Genetic defects 
-> Injury to gland 
-> Infection  
-> Tumors  

When a thyroid enlargement is found, it’s important to find the cause. At first, Thyroid Treatment should be done. Depends on the result of the thyroid function tests, your doctor will advise you to go for further tests like thyroid ultrasound, fine needle aspiration biopsy, or radioactive iodine scan.

-> Goitre causes due to iodine deficiency is treated with iodine supplementation. This may reduce the size of goiter, but does not totally resolve.
-> Goitre causes due to Hashimoto’s thyroiditis with Hypothyroidism Treatment  is treated with thyroid hormone replacement tablet. Here also goiter may become smaller, but does not completely resolve.
 -> Goitre causes due to Grave’s disease can be treated with radioactive iodine. In these cases goitre may disappear.
-> Multi nodular goiters with normal thyroid hormone level do not require specific treatment. But these cases are in risk for hypothyroidism or hyperthyroidism in future.
 -> If the goitre is too big and obstructs the airway, your doctor may suggest surgical removal of goitre.
 -> Regular check-up and regular monitoring of thyroid function are very important in case of all goiters.
